We’re learning more and more private-duty agencies are turning down in home visits due to limited (personal protection equipment (PPE), and clients are refusing visits worried caregivers may carry the deadly virus into the home.

At Connected Home Living, loved-ones are provided:

  • A Remote Care Coordinator (RCC) specifically trained on Chronic Care Management and COVID-19
  • Latest intuitive Telehealth solution customized for every client
  • Proactive check-ins and reminders
  • Rapid access to live board-certified RNs and Physicians*
  • Assistance obtaining medication and food
  • Companionship to combat loneliness

How it works:

  • Sign-up loved-one’s based on their needs and technological ability using a smartphone, tablet, computer or landline
  • CHL develops a care plan and trains your loved-ones to use our telehealth solution
  • Daily Health Surveys monitor your health
  • Live Remote Care Coordinators (RCCs) evaluate your needs and provide support
  • RCCs escalate issues to a Family Member or Nurse, based on pre-defined protocols
  • Nurse or Physicians may assess and treat patient using video triage*
  • RCC can coordinate medication, food, and supplies

Initial sign-up for minimum of 2 months of service (non-refundable). Month-to-month billing for services thereafter. You can cancel any time after first 60 days of service.

*Access to virtual visits with clients own Physician or Home Health Nurse is included in the Platform if MD/RN agree to participate.
CHL can also provide private-pay Registered Nurses and Physicians at an additional cost.
